Petroleum and Natural Gas Sales Forms

					ETS Account Set Up/Change Form You must have an account on the department's Electronic Transaction System (ETS) in order to submit posting requests for petroleum and natural gas rights. Use this form to set up a new account or to change an existing account.
					Direct Purchase -  Statutory Declaration An application for the Direct Purchase of P&NG rights must be supported by a statutory declaration stating that you or your company have ownership of or control over the P&NG rights in the freehold portion of the spacing unit.
					EFT Authorization - Sample The method of payment for both the public offering and direct purchases is by Electronic Funds Transfer (EFT). To register for EFT, an authorization letter must be submitted to the department.

Tenure Administration Forms

General

					Authorization Form Use this form to authorize another party to apply for validation, grouping and continuation on behalf of the designated representative, to authorize another party to request surrender of an agreement, or to authorize confidential use of your company's data to support another party's application.
					Notice of Official Service Address Land Tenure Clients must provide the department with written notification of their official service address.
					Notice of Designation, Replacement or Revocation of Representative The department must be notified of any changes to the designated representative in relation to a mineral agreement.
					Certified Copy of Document - Request This form is used when the lessee(s) or the designated representative requires a copy of the original document of a mineral agreement when their copy has been lost.

Transfers

					ETS Account Set Up/Change Form You must have an account on the department's Electronic Transaction System (ETS) in order to submit a transfer. Use this form to set up a new account or to change an existing account.
					Transfer Form Transfers of registered interest in Crown P&NG and oil sands agreements must be submitted electronically using the Electronic Transfer System (ETS). The transfer form is used to process the manual submission of a transfer. The department accepts manual submissions when only a part of the location contained in the agreement is being transferred or when special circumstances warrant it.
					Transfer Transmittal Letter This letter accompanies the transfer form when transfers are manually submitted.
					Transfer Form - Federal SSB (Soldier Settlement Board) Lease Transfers of registered interest in a Federal SSB lease cannot be submitted electronically using the Electronic Transfer System (ETS). This transfer form is used to process the manual submission of a transfer for a Federal SSB lease.
					Transfer Transmittal Letter - Federal SSB (Soldier Settlement Board) Lease This letter accompanies the transfer form for a Federal SSB lease when a transfer is manually submitted.

Security Notice

					Security Notice This form is used to register a security notice.
					Security Notice/Discharge Transmittal Letter This letter accompanies the submission of a security notice, notice of discharge or partial discharge and notice of assignment or partial assignment of security interest.
					Notice of Discharge or Partial Discharge of Security Interest This form is used to either discharge a registered security notice relating to all agreements against which it is registered or partially discharge a security notice as to some but not all agreements affected by the security notice.
					Notice of Assignment or Partial Assignment of Security Interest This form is used to register the assignment or partial assignment of a security notice.
					Notice of Change of Address for Service for Secured Party The secured party must provide the department with notification of a change to the address for service of the secured party.

Surrender

Notice of Surrender Use this form to surrender a petroleum and natural gas lease or licence, or portions of a lease or licence. This form must be completed by the designated representative, or an individual authorized by the designated representative.

Builders' Lien

Builders' Lien This form is used to register a Builders' Lien.

 

Builders' Liens/Discharge Transmittal Letter This letter accompanies the Builders' Lien when submitting a Builders' Lien for registration.

                      

Discharge of Lien This form is used to either discharge a registered Builders' Lien relation to all agreements against which it is registered or partially discharge a Builders' Lien as to some but not all agreements affected by the Builder's Lien.

Encumbrance

Certified Copy of Encumbrance Request This form is used to request a certified copy of an Encumbrance.

Agreement Continuation, Validation and Grouping

					Licence Grouping Application Use this form when applying to group two or more existing Petroleum and Natural Gas Licences in their initial term. For more information, please refer to the Licence Grouping Guide (PDF) and Initial Term Licence Frequently Asked Questions (HTML).
					Licence Validation Application. Use this form when applying to validate an existing Petroleum and Natural Gas Lease or Licence based on a validating well. For more information, please refer to the Licence Validation Guide (PDF) and Initial Term Licence Frequently Asked Questions (HTML).
					Continuation Application Form Use this form when applying for continuation of an existing Petroleum and Natural Gas Lease, or a Petroleum and Natural Gas Licence in its intermediate term. 				n/a	Requests for Review. To request a review of a continuation decision received from the Department, or to request a non-productivity review of a third party agreement, please send a letter to the Continuations area, Tenure Business Unit, Department of Energy. 					Continuation - Statutory Declaration for Crown and Freehold Rights in Spacing Unit An application for the continuation of a Petroleum and Natural Gas Lease must be supported by a Statutory Declaration stating that you or your company have ownership of or control over the P&NG rights in the freehold portion of the spacing unit.

Well Administration & Undisposed Rights

					Request for Well Re-entry Authorization (Revised June 09) This form is to be used when applying to re-enter an abandoned well bore that has vested to the Crown.
					Request authorization for operations in Road Allowance This form is to be used when applying to access Crown minerals within a statutory road allowance as per Information Letter 2011-09.
					Request authorization for operations in Undisposed Crown mineral rights This form is to be used when applying to conduct operations in undisposed Crown mineral rights.

Offsets

Offset Notice Response This form is used to respond to the six-month notice to indicate the option selected to resolve the offset obilgation resulting from a producing freehold well which adjoins the Crown spacing unit. Offset Response Guide (PDF)
