Coal Leasing Forms
The following forms are to be used when applying for a coal lease or requesting changes.
		Coal Lease Application Form This form is to be used when applying for a Crown coal lease or a coal lease in a road allowance. Please include fee with application. You may drop off your application at the Coal and Mineral Development Business Unit in Edmonton or the Information Centre in Calgary. Or you may fax or mail your application to the Business Unit (see Contacts.) For more information see Coal Reporting Instructions.
		Change in Lease Ownership Request Form. If you are unable to request a change using the E-Transfers service (see Online Services), you may use this form. The Transmittal Letter below should accompany the completed form. Send your request to the address at the bottom of the form. For more information see Instructions.
		Change in Lease Ownership Transmittal Letter. When applying for a change in lease ownership using the above form, please complete and attach this letter. Send your request to the address at the bottom of the form. For more information see Instructions.
	N/A	Coal Lease Amendment Request. There is no form. Please write to Coal and Mineral Development Business Unit (see Contacts), giving the lease number and the requested changes to the land description required.
	N/A	Coal Lease Cancellation Request. There is no form. Please write to Coal and Mineral Development Business Unit (see Contacts), specifying the lease to be cancelled and the requested effective date.
	N/A	Coal Mining. For development of Crown and freehold coal. Contact the Energy Resources Conservation Board regarding forms and procedures relating to coal mine permit areas, coal mine licenses, coal projects, and coal mine abandonment.

Coal Royalty Forms
The following forms are to be used for reporting production and calculating royalty on Alberta-owned coal produced in the location of a coal lease.
		Coal 1. Sub-Bituminous Coal Monthly Production Report for Crown coal production, due on or before the last day of themonth following production month.
		Coal 2. Sub-Bituminous Coal Estimated Annual Production Report for Crown coal production, due on or before Dec. 31 before upcoming calendar year.
		Coal 3. Bituminous Coal Monthly Royalty Report for Crown coal production, due on or before the last day of the month following the production month.
		Coal 4. Bituminous Coal Estimated Annual Royalty Report for Crown coal production, due on or before December 31 before the upcoming production calendar year.
		Coal 5. Bituminous Coal Annual Royalty Report and Schedules for Crown coal production, due on or before March 31 following the year of production.
